This is a Logitech QuickCam 4000 webcam, barely used and not modified.
I opened the case and found some good news, hence the higher-than-usual price for this model:
QC4000's can be found with any of 3 different CCD imaging chips inside: the Sony ICX098BQ, Sony ICX098AK, or Sharp LZ24BP. By the luck of the draw this one has the Sony BQ CCD, which has the highest sensitivity and lowest noise levels of the three. (It's the same CCD as in the Toucam Pro 740, which most people consider to be the best budget webcam out there for astrophotography).
This camera can be used as-is or can be modified for long-exposure a la Steve Chambers.
Price is $75, and includes shipping CONUS.
