I had to make mine, using aluminum tubing, an electrical outlet box and a piece of flooring, instead of a wooden plate.
You could make it from whatever parts you can find that will work. As for attachment, well I used two blocks of wood with a holes for the tubing on a wooden plank and then a bolt through it into the rocker box.

Yes, this was a juction box, used it as if I were running conduit. Attached the plate and a piece of angle aluminum as a shelf at the bottom and that was it.

The parts were all from Lowes, so easily purchased. You can just run the original cables up the tube or just use them normally and let them dangle, which would be pretty easy.
