Fork mount

Several years ago I made a 12.5 inch f/7 telescope. I mounted it on a very heavy pipe mount. I want to re-mount the OTA on a permanent fork mount in my back yard. Does anyone have plans for a fork mount that they can share with me for this behemoth?
