That's like saying diabetic patients who died with heart disease died with diabetes...
"Deaths among people with COVID-19 and deaths caused by COVID-19" seems like a reasonable and medically important distinction. Obviously, if the health care system plays politics with this it messes up the data. I personally think that the lion's share of doctors, coroners and the like are too busy to play politics. Even in South Dakota. Who knows what the bureaucrats will do later, if they have the means to adjust the data one way or another.
A personal acquaintance recently lost his father in law. The poor old chap had late stage untreatable lung cancer and was in the end stages of home hospice. Palliative care only, DNR and so on. The father-in-law's wife and daughter were scrupulously distancing with delivered groceries and no outside contact except for hospice care as they waited for the inevitable. The end was going to be a few days away, at most.
Sure enough, a visiting hospice worker infected the whole family. The father in law died at home the day after his positive test result. They would have never tested him, but the wife and daughter got sick too, they are now recovered.
This happened in San Diego county. I have no idea how his death was recorded, but if it is listed in any fashion as a Covid death, then grim Cancer has been deprived of one rightful victim.
