EQ mount repairs ?

I have a bent worm gear shaft.
Seriously, does anyone have a machine shop they trust, or preferably have used in the recent past?

The shaft is bent as it comes through the holder, looks like it got dropped or knocked over. The motor drive gear now hopelessly precesses.
